Is there a specific document(s) a doctor has to to fill out for your physical exam and vaccinations if you'll be doing rotations in NY, or will any standard document from the doctor's office showing proof of your physical exam and vaccinations be OK?

Most NY hospitals want titers, proof of vaccinations. A PPD and physical exam done within the last year in order to matriculate at their hospitals.

Is proof of vaccinations enough, or do you need titers too?
Also, will they let you start if you haven't had all 3 series of hepatitis B vaccinations?

you need actual titers, records are not sufficient.

Obviously, you should call and/or check the website for any new hospital you will rotate at. I was really glad I read through the stuff for Wyckoff. As I recall they want titers for mumps, mealses, rubella AND varicella (chicken pox)! in addition to the physical. Students who don't have this stuff can get it in a two hour-a-day lecture, a Wyckoff "elective", but will not be doing any core rotations with the big boys and girls, and will lose their spot.
