A 23 year old lady comes to your office because she feels very tired, she also states that her menstrual flow is very heavy, she is not taking any vitamin supplements, She has a Htc of 29 and a hemoglobin of 9, her MCV is 70.

What is the most accurate test to Dx this patient?

A- ferritin levels
B- RDW
C- WBC
D- Tommy k(We miss you)
E- Bone marrow Biopsy

I think its A........Based on the history it seems like iron deficiency.

you are correct that it's iron deficiency, but ferritin levels are not the most accurate test.

just to add. it's called effective erythropoiesis, which is the proper response of BM to respond to anemia.

what is ineffective erythropoiesis and where is it seen?

i'm assuming you check the bone marrow to see if its aplastic anemia? well in that case, isnt it a normocytic anemia with a corrected reticuloycte count < 3%? This case has microcytic RBCs. So I woudl say ferritin levels as all signs point to iron def. anemia. Any other explanations?

there is no new synthesis in the marrow to tell it is micro or macro the old Rbc in the circulation are normocytic and when the complete life span of 120 days even if very small quantity of Rbc are formed there is no shortage of iron but rather there is iron from the cells destroyed as well so dear it is jst the appearance of bone marrow that would tell u abt the aplastic aneamia and u know first imp complication u r going to handle is lack of platelets.tc
