Kaplan : P.ANCA in polyarteritis nodosa


FA : no ANCA in PAN
P.ANCA in microscopic polyangiitis only according to new reseraches!!

In polyarteritis nodosa - p-anca and c-anca, but mostly p-anca.

In Wegener's granulomatosis - c-anca only.

p-anca is not just for PAN. In that sense, they're not of diagnostic value but does help to narrow it down or to differentiate. Also look for other lab values such as increased ESR and 30% chance of Hypergammaglobulinemia in PAN. also, hep. B surface antigen in 30% of PAN pxts.

If you happen to see anca in the q. stem.. you can narrow your thoughts to "autoimmune diseases or autoantibodies".. then rely on other symptoms and signs to pick out the disease.

PAN is taught in review books that it is associated with P-ANCA although big Robbins pathology text book says that it isn't associated with P-ANCA (the newer edition, the older edition says it is associated)

here is wat goljan rapid review states:

p-ANCA: wegener's granulomatosis

c-ANCA: microscopic polyangitis, Churgh Strauss

it dsnt state any ANCA for PAN
