What is it exactly?

I know it deals with working with athletes etc

1) It can be a rotation during an orthopedic surgery residency or a fellowship after it is done:

2)It can also be a primary care fellowship (i.e., done after a residency):

Here is an explanation of what the fellowship entails:
The goal of the Montana Family Medicine Sports Medicine Fellowship is to produce primary care physicians who are excellent in their ability to manage patients with musculo-skeletal problems as well as general medical problems as they pertain to athletics and exercise. Additionally, the fellow will design and complete a primary research project in sports medicine and be responsible for teaching the sports medicine curriculum to the Family Medicine residents.*
