The diuretic causes hypokalemia... this is bad if your taking digoxin b/c digoxin and K+ compete for the same site on the Na+/K+ ATPase... you'll get digoxin toxicity...

for some reason i had a completely different understanding of this concept...not the diuretic part .dig blocking part...if you have a few minutes please explain it.if you have low pottasium how do u compete???or did i miss the boat completely ?sorry for making you think... shame on me. ;) ;) ;)

If digoxin and K+ compete for the same ATPase, then in case of hypokalemia, K+ is actively using its ATPase to get excreted. Digoxin therefore cannot use it, and stays in the system...I think. Someone help me out if I am thinking about this the wrong way.

If you have hypokalemia, there will be increased Digoxin binding to Na+/K+ ATPase (b/c there's less K+ competing with Digoxin). This will enhance Digoxin toxicity.

I just realized it and wanted to share...MOA of acetazolamide in altitude sickness:create a metabolic acidosis ,so compensate the respiratory alkalosis (hyperventilation) of high altitude.

that's right:D , if K+ and digoxin both compete for Na+/K+ATPase ,in hypokalemic situation, K+ is out of competition since he's low,then digoxin has free access and win the race, the same way,hyperkalemia reduces the effects of digoxin.
My Question:what is the MOA of using thiazides in nephrogenic diabetes insipidus?:confused:
