What is the most common primary immunodeficiency?









































































The most common primary immunodeficiency is IgA deficiency considered when IgA levels are less than 5 mg/dl. There are two isotypes of IgA: IgA 1 found in the serum; and IgA 2 found in the mucosal secretions as a dimer with bound together by a J chain with a secretory piece attached.

IgD = Found as a receptor on B cells

IgE = binds to Fc epsilon receptors on mast cells and basophils and is involved in hypersensitivity rx type I.

If you have low levels of IgA what other Ig WILL GO UP TO COMPENSATE?

I might add the following. Pts IgG subclass deficiency usually live a normal life span. A potentially clinically important point is that some patients with IgG2 deficiency also have a IgA deficiency and may develop anaphylaxis if given Ig-A containing blood products.

Yes, remember that IgA willl try to stop the bad guys to attach, so if he fails or is absent who is the first to come up IgM, also remember that IgM is for acute where IgG is for chronic.

Now that J chain I will have to review it because I know its there but dont remember much about it.
