How practical is it to be able to set up your own AGME cores? Do you ask S***** first or do you seek out potential hospitals first?

make contacts early by email or use ones you already have. www.acgme.org (http://www.acgme.org/)
go through sandy first and make sure you have all your paperwork organzied.
make a compelling case for why they should take you, ie, you are a local, you get top grades, you have won awards, you do community service, etc.
I set up my own in Maine, except surg, through this method.

Is it very hard to set up your own acgme core rotations? I'm from TN and would like to get something close to or in TN.

hey, I'm from TN too!
I know ETSU's program will allow FMGs for a bit, especially if you're from the Tricities area. However two of my classmates made their way in and have been doing them all there. I talked to the woman about doing some rotations there myself (since I used to work at the medical center there and attended ETSU) and she was going to send me a packet - but I never got it! :( She told me, however, that she doesn't set up the rotations for you. She sends you a list of the doctors and then it's up to you to contact them and set up the rotation with them.
I don't know of any other place in TN that Saba students have rotated, but I'm sure you can find some.

By the way, the "no comment" thing is a loooooooooong story pertaining to Scoobz. Just make sure our clinical coordinator in Gardner, Mass approves any and all rotations you set up.

Well that's awesome. I hope you get to do yours there and maybe you can help me make my way in. I am so happy to hear that. What semester are you swinginisland doc? I am starting Jan 07.

one of the dean's, dr. duckworth, is a surgeon from tenn. he could be very helpful to you. also, his father is the head of a path residency program in tenn. he is always eager to help up and coming students, so impress him and go from there.

I actually talked to the woman (Sandy in the clinical office of Saba can give you the info) and she was all excited about me coming, was going to send me the info and mark the physicians who had worked with Saba students before - but I never got it. *shrug* Oh well. I'm enjoying seeing other areas.
ETSU also takes IMGs for residency. So another option could be to travel for clinicals, see several different hospitals, and apply for residency in TN.
I'm about to start my fourth year rotations. Actually in 5 days I take Step 2. Best of luck on Saba! When you start I'll be just about done. ;) (Finally!)

having recently setup my own cores, i can tell how the procedure goes with St Matthews, i am sure it will be the same at Saba.
the first thing i did was approach the hospital and talk to them about a possible spot for cores. i did this about 10 months prior to my completion of 2nd year (about 2 months into 2yr). even this early the cores were full. persistence and luck got me a spot at the hospital so do the majority of my cores, and once they had accepted me, i contacted my uni and they green lighted it.
the two most important things really in doing this are
1 - plan WELL ahead of time. like seriously a year is good. also most hospitals have ties with US med schools and won't take any nonLCME med students until year 4. your best bet is to contact private hospitals that already take foreign med students.
2 - get the rotation setup with the hospital BEFORE you talk to your school. there is no point getting excited until you have everything in place.

if u hate everyone u go to school with and would like to be in a new environment its great. also if you want to set something up in a particular location its great. if u want a little of both, its perfect

For students with children, it would be nice to stay in one location! I'm making contacts for my cores and electives now, before I even hit the island.

You can't do your own cores - not unless you can convince the hospital you want to do them at to add Saba students permanently - someone in our class tried to set his own up - they did everything they were told to do and when they needed the people at Gardner to sign on the dotted line - nada! Don't expect it to be any different for anyone else - they may tell you you can, but in the long run, it wont happen!
