A urine Specific gravity greater than 1.023 in the first morning void correlates BEST with a


1-normal dilution
2-tubular defect
3-negative free water cleareance
4-possitive free water clearance









This is a NEGATIVE FREE WATER CLEARANCE, wich means that the concentrating mechanism is intact. In the ascending Limb, FREE WATER is generated by the presence of an active chloride pump. When ADH is supressed the FREE WATER is lost, producing a possitive free water clearance. This represents normal dilution. When ADH is present however the free water is reabsorbed from the late distal tubule and collecting duct, thus producing a negative free water clearance and a concentrated urine.
