Coming from India it would be hard to get into a UK school or HO year due to new immigration laws aimed at giving UK citizens and EU member nations first crack at jobs and training. If your brother wants to pursue medicine in the US he might be better off doing his bachelors (4 yr undergraduate work) in the US and applying for US medical school after.

US schools don't give out MBBS degrees all medical studies are at the graduate level and require undergraduate degrees.
