In the American system of medical education the professional qualification is an M.D. (doctor of medicine) which usually takes only 4 years. However, the student is required to have a Bachelor's degree already. This can be in any subject - provided that some short basic science courses have been taken at an approved college.
The Indian (and British etc. etc.) MBBS takes 5 or 6 years. If your brother has an MBBS from his own country he can sit for the USMLE (the American licensing exam that anyone wanting to practise there must take) and he doesn't need another bachelor's degree.
