Consider applying to US programs that are on the border with Canada. E.g, Michigan. There are trainees who live in Ontario, cross the border daily or weekly to Michigan. They maintain residence in Canada, but work in MI. Your spouse/family could still live in Canada, and you would be able to train/work in the US. While you are in residency , you make sure you keep up with licensing requirements, and get a licence in Canada.
