The ECFMG website states the doctor can stay in the US for the length of time needed to complete a ACGME residency. Extensions are allowed, for ACGME subspecialties/chief resident/fellowships, up to a maximum of seven years. Some fellowships can be 'non-standard' i.e., non-ACGME. Extensions have to be approved by the home country. Note that doctors are not entitled to the maximum of seven years.
