I think the differences of KMC and KMCIC are more a matter of priorities and personal motivation.
Priorities:
Do you want to complete Indian licensing requirements in Medical School and eventually practice in India? If so KMC would be a great option for you. Do you want to complete US licensing requirements in Medical School and eventually practice in the US? KMCIC would meet you needs better than KMC.
Personal Motivation:
From what KMCIC students have told me, I don't think any student right out of high school who is still not sure she wants to be a doctor should go to KMCIC. Why? KMCIC is in the middle of rural India 10,000 miles from the US or Canada. If there is a possibility that you will be distracted away from your medical studies, Manipal is not where you should be. KMC's curriculum I imagine gives a little less time for their students to be distracted. Don't know for sure. But this is my impression.
My suggestion is to test your motivation to be a doctor: volunteer in the ER, work for a while in the front office, shadow a doctor on rounds. If everything in you is still pointing you toward medicine it's only a matter of applying yourself to find the right path.
