No, you can't write before your fifth year. You can write in spring of your fifth year, i.e., during your fellowship. The RCPSC won't allow you to write until it has agreed you have met its requirements by doing a four year program and a one year fellowship.

Canadians in residency can write their exams in spring of their final year, with the assumption they will finish their programs in June. Similarly, you can write in your fifth and final year.

Remember you will do the Preliminary Assessment by april 30 of your PGY4, in order to write the exam one year later. The RCPSC will issue a ruling letter in December, while you are in the fellowship. You will have to tell them for the Assessment that you are doing a fellowship. The ruling letter will allow you the first opportunity to write the exam in spring of your PGY5. Planning. Planning.
