A typical action plan for psychiatry would be:

4 years ACGME accredited adult psychiatry
1 year ACGME accredited fellowship
or
3 or 4 years adult psychiatry
2 years child and adolescent psychiatry
apply for preliminary assessment by april 30 of the year before you will write the RCPSC exam in psychiatry, i.e., by april 30 of PGY4
pass the MCC QE I and II by end of PGY3
request extension of statement of need to change to fellowship
write RCPSC exam in spring of PGY5 (i.e, in final year in US
write American board exam after residency is finished
apply for licence in Canada / seek job in Canada

It doesn't need to be any more complicated than that.
