I edited my post to answer your first question

2) Other credentials, do what interests you and if it involves an organization...great, if it doesn't, put it down under hobbies. Just so they know you are more than a bookworm. I used my extracurriculars from undergrad and medschool, focused more on med school
3) I used the same letters i used to for ross
